Missing Your ID Card?

State employee or non-Medicare eligible retiree group participant

To request a new COVA Care or COVA HDHP (High Deductible Health Plan) ID card: 
  • Step 1. To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct. 
  • Step 2. If your mailing address is correct: Call the ID Card Order Line at 1-866-587-6713 . Provide either your health card ID number or your social security number. A new ID card (two with subscriber plus one) will be sent to you within seven to 10 business days.
  • Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.
To request a new COVAHealthAware ID card:
  • Step 1. To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct.
  • Step 2. If your mailing address is correct: Log in to your Aetna member website, select the link to request a new ID card, and print a temporary ID card for use in the interim. Or call Aetna Concierge at 1-855-414-1901. Provide your name and date or birth or member ID number (employee ID preceded by two zeroes). New ID cards will be sent to you within seven to 10 business days.
  • Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 3 business days and follow Step 2.
To request a Kaiser Permanente HMO ID card:
  • Step 1. To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct.
  • Step 2. If your mailing address is correct: Contact Kaiser directly at 1-800-777-7902 or (301) 468-6000. Your card(s) will be mailed to you, usually within seven to 10 business days. You can also view your ID card securely on the Kaiser mobile app. After logging in, you can view your member ID card at the bottom of the home screen. 
  • Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 48 hours and follow Step 2.

To request a new Optima Health ID card:
  • Step 1. To be mailed an ID card, your mailing address must be up to date in the Commonwealth of Virginia's system. Visit EmployeeDirect or contact your agency Benefits Administrator in the Human Resources Department to be sure that your mailing address is correct.
  • Step 2. If your mailing address is correct: Visit optimahealth.com/cova or download the Optima Health mobile app where registered users may sign-in to their secure portal and select the ID Card option to request a new or replacement card. The Optima Health mobile app also offers registered users the ability to view their ID card electronically.  Or members may call Member Services at 1-866-846-2682 to request a new ID card.  Your card(s) will be mailed to you, usually within seven to 10 business days.
  • Step 3. If your mailing address is incorrect: After taking the action in Step 1, wait 3 business days and follow Step 2.

 

Extended Coverage Participant

To request a new COVA Care or COVA HDHP (High Deductible Health Plan) ID card:
